Responsibilities:<ul><li>Provide direct nursing care to hospice patients in their homes.</li><li>Make home visits under the supervision of an RN Case Manager.</li><li>Assess and monitor patients' physical and emotional needs.</li><li>Perform nursing interventions according to the hospice care plan.</li><li>Educate patients and families regarding hospice and end-of-life care.</li><li>Communicate changes in patient condition to the RN and care team.</li><li>Assist the RN and medical provider with specialized end-of-life care.</li><li>Document and report patient responses and changes in condition.</li><li>Collaborate with the interdisciplinary hospice team.</li></ul>Requirements:<ul><li>Active, unencumbered LPN license in Vermont and New Hampshire OR compact LPN license.</li><li>Graduate of an approved practical nursing program.</li><li>Experience in medical/surgical nursing and/or home health care.</li><li>Hospice experience is beneficial.</li><li>Ability to work independently and make home visits.</li><li>Strong communication and patient/family education skills.</li></ul>
