Description
RN – Home Health
Job Type: Contract / Travel
Contract Length: 12 Weeks
Schedule: 8-hour shifts
Guaranteed Hours: 40 hours/week
Position Overview
We are seeking an experienced Registered Nurse (RN) – Home Health to provide skilled nursing care to patients in their homes. The RN will be responsible for patient assessment, care planning, clinical interventions, patient education, and coordination of care with the interdisciplinary team.
Key Responsibilities
- Perform comprehensive home health and hospice patient assessments.
- Develop, implement, document, and evaluate individualized Plans of Care (POC).
- Provide skilled nursing services, including wound care, IV care, venipuncture, phlebotomy, and catheter/drain management.
- Administer medications and monitor patient responses.
- Educate patients and families on medications, treatment plans, diet, safety, and self-care.
- Assess and manage pain and other clinical needs.
- Coordinate care with physicians, therapists, LVNs, HHAs, and other healthcare professionals.
- Provide clinical direction and supervision to HHA/STNA/LVN staff as applicable.
- Maintain accurate and timely patient records and documentation.
- Monitor assigned cases for compliance with third-party payer requirements.
- Participate in interdisciplinary team meetings and case conferences.
- Provide on-call services as assigned.
- Follow infection-control and OSHA requirements.
- Travel locally between patient visits and work independently in the home setting.
Required Qualifications
- Active RN license
- 2+ years of Home Health experience
- 1–2 years of clinical RN experience
- OASIS experience – Required
- Homecare Homebase (HCHB) experience – Required
- Current CPR/BLS certification
- Valid driver's license
- Reliable transportation and current auto insurance
- Graduate of an accredited ADN or Nursing Diploma program
Preferred Qualifications
- BSN
- CHPN certification
- Experience with wound care, IV therapy, venipuncture, phlebotomy, and catheter/drain management
- Strong care coordination and patient education skills
