Physical Therapist – Home Health | Day Shift
Shift: Day 5x8-Hour (8:00 AM–4:30 PM)
Guaranteed Hours: Not specified
Contract: 13 weeks
Job Overview
- Location: Kauaʻi, Hawaii
- Position: Physical Therapist – Home Health
- Schedule: 5x8-hour days
- First-time travelers: Accepted
Required Experience
- Minimum 1 year of Home Health PT experience – REQUIRED
- Home Health experience required
- OASIS experience required
- Experience with Start of Care (SOC) and Medicare patients
- Must be able to provide:
- Total months of SOC/OASIS experience
- Frequency of Medicare SOCs per week
- First-time travelers are accepted.
Required License & Certification
- Active Hawaii PT License – REQUIRED and must be in hand at submission
- BLS – AHA REQUIRED
Submission Requirements
- Updated resume
- Hawaii PT License in hand
- AHA BLS
- Confirm Home Health experience
- Confirm OASIS/SOC experience
- Provide total months of SOC/OASIS experience
- Provide average number of Medicare SOCs per week
- Confirm willingness to occasionally float to other Hawaiian islands
- Include RTO at submission
