Certified Occupational Therapy Assistant (COTA)
Location: Montrose, NY 10548
Employment: Full-time, Onsite, W-2
Hours: 40 hours/week
Schedule: 7:00 AM–4:00 PM; rotating weekends/holidays
Start Date: September 2026

Position Summary:
- Provide occupational therapy interventions under the supervision of a licensed Occupational Therapist and Director of Rehabilitation Services.
- Help residents maintain or regain daily living, adaptive, and functional skills.
- Implement established occupational therapy plans through ADL training, purposeful activities, therapeutic exercises, and adaptive techniques.
- Support upper-extremity function, fine-motor skills, cognition, safety awareness, positioning, and functional independence.
- Assist with wheelchair/seating, power mobility, hand orthotics, adaptive equipment, and discharge-related programs.
- Educate residents and caregivers on assigned strategies, equipment, and safe performance of daily tasks.
- Monitor resident response and report changes, concerns, and progress to the supervising Occupational Therapist.
- Maintain treatment areas, sanitize equipment, and complete accurate treatment and progress documentation.
Requirements:
- Current, unrestricted New York State Occupational Therapy Assistant license.
- Minimum 1 year of supervised Occupational Therapy Assistant experience.
- Skilled nursing, long-term care, geriatric, rehabilitation, ADL, wheelchair/seating, or memory-care experience preferred.
- Facility credentialing required, including proof of license/credentials, current physical examination, and TB/PPD documentation.
- Maintain resident confidentiality and complete timely, accurate documentation in the facility EHR.
- Ability to work effectively with nursing, medical, social services, residents, families, VA, and outside equipment vendors.
