Description
Pride Health is seeking a Physical Therapist (PT) to join our team in San Jose, CA 95125. This is a 3-month contract with potential for extension. This position provides restorative and rehabilitative Physical Therapy services to elderly participants in both center-based and home settings.
Job Responsibilities:
- Conduct Physical Therapy assessments and develop individualized participant care plans.
- Provide Physical Therapy interventions including therapeutic exercises, gait and functional training, muscle re-education, and ADL training.
- Provide therapy services at the center and in participants’ homes based on their needs.
- Participate in interdisciplinary team (IDT) meetings and care coordination.
- Monitor participant progress and communicate changes in condition to the IDT.
- Educate participants, caregivers, and staff on therapeutic equipment, safety, and functional activities.
- Supervise, train, and mentor Therapy Assistants and other direct reports.
- Coordinate care transitions between hospitals, skilled nursing facilities, homes, and other settings.
- Maintain accurate and timely clinical documentation.
- Support quality improvement initiatives, staff training, and team meetings.
Required Qualifications:
- 5 years of progressively responsible Physical Therapy experience.
- Minimum 2 years of clinical Physical Therapy experience with frail or elderly populations.
- Experience treating elderly patients with complex medical and functional needs.
- 2 years of supervisory experience with the ability to mentor and develop team members.
- Strong clinical, leadership, organizational, and communication skills.
- Ability to work independently and prioritize in a fast-paced environment.
- Experience working with diverse populations and interdisciplinary teams.
- Bachelor’s Degree in Physical Therapy required.
- Valid California Physical Therapy License required.
- BLS certification required.
- Work Arrangement: Combination of onsite and mobile services.
- Home Visits: 1–2 days per week dedicated to home safety assessments and therapy needs.
- Reliable transportation required.
- PT will participate in interdisciplinary meetings, care coordination, and direct patient care.
Additional Information:
- Location: San Jose, CA 95125
- Schedule: 40 hours/week - Monday–Friday, 8:00 AM–4:30 PM
- Contract Duration: 3 Months
- Weekly Pay: $3,228/week
- W2 Pay: $31/hour
- Weekly Stipend: $1,988/week
- Local pay: $60 - $70/hour
*Offered pay rate will be based on education, experience, and healthcare credentials.
