Certified Nursing Assistant (CNA) – Long Term Care (LTC)

Position Requirements

Start Requirement

• Must be able to start within the next week
• Active NY CNA license (or proof of application with applied date) is required at the time of submission

Schedule

• 80 hours biweekly
• Monday–Sunday work week
• Rotating schedule: one week may be 32 hours, the next 48 hours
• Every other weekend required (minimum)
• No call
• No block schedules for any reason

Required Qualifications

• Minimum 1 year of CNA experience
• Long Term Care (LTC) experience required
• Current AHA BLS certification
• NY CNA license or proof of NY license application

Background Check Requirements

• 7-year background check completed within 18 months of the start date, including:
  • New York State check
  • Nationwide criminal check
  • County court searches
  • FACIS Level 3
  • Sex Offender Registry
• FACIS Level 3 and NY State check must be completed within 30 days of the start date

Submission Requirements

• Explain all employment gaps over 90 days
• Complete work history is required, including:
  • Bed count
  • EMR/charting system
  • Facility type
  • Unit details
• IVS required prior to offer

Travel & Local Rules

• Travel: 101+ miles
• Radius: 51–100 miles
• Local: 50 miles or less
• Mileage is measured by straight-line distance, not driving distance

Important Notes

• Public transportation is not accessible; reliable personal or rental transportation is strongly recommended.
• Vehicles should be equipped with all-season or snow tires during November–April.
