Registered Nurse Case Manager – Sitka, AK

Location: Sitka, AK
Start Date: 09/28/2026
Contract Length: 13 Weeks
Shift: Monday – Friday, 8:00 AM – 5:00 PM
 

IMPORTANT REQUIREMENTS:

  • Must have an active Alaska RN license at the time of submission
  • Flu vaccination is required
  • BLS Certification required
  • Case Management Certification preferred

Position Overview:
We are seeking an experienced Registered Nurse Case Manager for a 13-week assignment in Sitka, AK. The RN will provide comprehensive, patient-centered care in an ambulatory clinic setting, supporting patients and families across the continuum of care. This role focuses on care coordination, patient education, clinical assessment, and collaboration with providers and interdisciplinary teams.

Key Responsibilities:

  • Perform comprehensive patient assessments and develop individualized plans of care.
  • Implement and evaluate nursing interventions and patient responses.
  • Administer medications and treatments safely and accurately according to provider orders and facility policies.
  • Recognize changes in patient condition and initiate appropriate interventions or escalation of care.
  • Provide patient and family education regarding medications, treatment plans, and discharge needs.
  • Coordinate care with providers and interdisciplinary healthcare teams.
  • Promote quality outcomes, patient safety, and continuity of care.
  • Maintain accurate and timely documentation in the electronic medical record.
  • Follow infection control, regulatory, and organizational standards.
  • Provide culturally responsive and compassionate patient care.

Requirements:

  • Active, unrestricted Alaska RN License at time of submission
  • Graduate of an accredited nursing program
  • Bachelor’s degree preferred
  • Current BLS Certification
  • Minimum high school diploma or equivalent
  • Flu vaccination required
  • Case Management Certification preferred
  • Strong clinical assessment, communication, and care coordination skills
