Submission Requirements
Med Surg, Telemetry or Stepdown experience [required]
Some oncology experience is [required]
EPIC EMR experience [required]
BLS [required]
ACLS [required]
IL license required to submit, IA license apply upon

Profile should include

·             Driver’s License [required]

·        Work history/resume [required]

·        Skills checklist (must have been completed within 1 year of submission date) [required]

·        License verification [required]

·        Copy of required certifications

·        References - 1 supervisory reference required at time of submission [required]

**Must have taxable address outside of 50 miles from facility. **

Unless Madison where they will accept local with a 10% rate reduction.
