🌿 Travel Physical Therapist – Home Health | Medford, OR | Day Shift
Position: Physical Therapist – Home Health
Location: Medford, OR & Surrounding Areas
Start: ASAP
Contract: 13 weeks
Shift: 5×8-hour Days | 8:00 AM–4:30 PM
🔥 Must-Have Requirements
- 1+ year Skilled Home Health PT experience – Required
- BLS/BCLS – Required
- OASIS experience – Required
- Reliable personal transportation – Required
- First-time travelers not accepted
- Must be comfortable with 30 productivity points
- Must be able to visit up to 6 patients/day
🏠 Position Details
- 8-hour shifts only — 4×10s and 3×12s are not available
- Locals accepted within 50–75 miles at the same rate
💉 Other Requirements
- Primary COVID vaccination series plus 1 booster required
- Must have reliable transportation
- Must be willing to float between facilities within the state if required

- Previous Providence experience: Travel / PRN / Permanent
- Reliable transportation: Yes/No
- When and where OASIS experience was obtained
- Candidate's usual productivity point expectation
- 2 references covering at least 1 year within the last 3 years
