Physical Therapist – Home Health
Contract Duration: 14 Weeks
Coverage Area: Huntingdon County, Northern Blair County & Centre County
Schedule: Monday–Friday, 8:00 AM–4:30 PM | 40 hours/week
Mileage: Reimbursed between patient visits
Job Summary
Seeking a compassionate Physical Therapist to provide skilled physical therapy services to patients in their homes. The PT will evaluate patients, develop individualized treatment plans, provide therapeutic interventions, educate patients and caregivers, and document progress to promote safe mobility and independence.
Responsibilities
- Perform PT evaluations and develop individualized plans of care.
- Provide therapeutic exercises, gait, balance, and transfer training.
- Educate patients and caregivers on exercises, mobility, and fall prevention.
- Monitor patient progress and adjust treatment plans as needed.
- Coordinate care with physicians and the interdisciplinary team.
- Complete accurate and timely documentation.
Requirements
- Current Physical Therapist license.
- CPR/BLS certification.
- Valid driver’s license and reliable transportation.
- Strong communication, organization, and clinical skills.
Preferred: Home health and OASIS experience.
Dress Code: Navy blue scrubs/polo shirt with khaki pants.
