- Active Oregon PT license
- 1 year of skilled Home Health PT experience
- BLS
- OASIS experience
- Reliable personal transportation
- Lifting/transfer experience
- Ordering supplies
- Scheduling appointments
- Ability to see up to 6 patients/day
- Productivity expectation: 30 points
- Primary COVID vaccination required
