Job Title: LPN - LTC
Location: Clifton Springs, NY 14432
Shift: Mid  (15:00 - 23:00)
Shift Length: 5*8 hours
Guaranteed Hours: 40 hours per week
Duration: 13 weeks

Position Summary:
We are seeking an experienced Licensed Practical Nurse (LPN) with at least 2 years of recent Long-Term Care (LTC) experience.
The LPN will provide compassionate nursing care to residents while ensuring their safety, comfort, and well-being.
Responsibilities include medication administration, patient assessments, treatments, and maintaining accurate clinical documentation.
The ideal candidate should have strong communication, clinical judgment, and the ability to work effectively in a fast-paced LTC environment.

Weekend Requirement: Every Other Weekend

Roles & Responsibilities:

• Provide direct nursing care to LTC residents, including assessments, treatments, and routine monitoring.
• Administer medications and treatments safely according to physician orders and facility protocols.
• Monitor residents’ conditions and promptly report changes to the RN or healthcare provider.
• Maintain accurate clinical documentation and communicate effectively with residents, families, and the care team.

Skills:

• Strong Long-Term Care (LTC) nursing and resident assessment skills.
• Proficient in medication administration, treatments, and clinical documentation.
• Excellent patient care, communication, and interpersonal skills.
• Strong clinical judgment, teamwork, and ability to prioritize tasks effectively.
