Description

Job Title: Clinical Social Worker – Outpatient Hospice / Medical Social Worker

Location: Albany, Oregon
Department: Hospice / Behavioral Health
Contract: 13 Weeks
Shift: Days | 8:00 AM – 4:30 PM
Schedule: Monday–Friday | No Weekends
 

 Mandatory Qualifications for This Job

  • Oregon LMSW (Master’s-level Social Worker) license OR CSWA (Clinical Social Work Associate) license required.
  • Healthcare Provider Level BLS certification required.
  • Previous hospice experience is strongly preferred.

 Roles and Responsibilities

  • Provide psychosocial support and counseling to hospice patients and families.
  • Assess patient needs and develop individualized care plans.
  • Provide crisis intervention, grief, and bereavement support.
  • Coordinate care with nurses, physicians, chaplains, and interdisciplinary teams.
  • Conduct home and community visits, including SNFs and assisted living facilities.
  • Connect patients and families with community and social service resources.
  • Advocate for patient needs and support end-of-life care planning.
  • Maintain accurate clinical documentation and patient records.
  • Ensure compliance with hospice policies and professional standards.
  • Travel within the assigned service area for hospice visits.

This is a 13-week Clinical Social Worker contract in Albany, OR, working days from 8:00 AM–4:30 PM, Monday–Friday with no weekends. Candidates must have an active Oregon LMSW or CSWA license and Healthcare Provider BLS. Hospice experience is strongly preferred, while LCSW and hospital/medical social work experience are preferred. Reliable transportation, a valid driver’s license, and required auto insurance are also necessary for community/home visits.
