Description
🏠Social Worker – Hospice
Shift: Days | 8:00 AM–4:30 PM
Schedule: Monday–Friday, no weekends
Contract: 13 weeks

- Hospice experience strongly preferred
- Social work experience in a hospital or medical setting preferred
- Experience providing psychosocial support to patients and families
- Experience with end-of-life care and hospice services preferred
- Ability to work independently in home-based/community settings
- Must be comfortable traveling to patient residences, skilled nursing facilities, assisted living facilities, and adult foster homes

- Oregon LMSW (Master’s-level Social Worker) OR
- Oregon CSWA (Clinical Social Work Associate)
- BLS – Healthcare Provider level
- Oregon LCSWÂ
đźš— Additional Requirements
- Valid driver’s license required
Best candidate: Social Worker with hospice or medical social work experience, an active Oregon LMSW or CSWA, BLS, and the ability to travel independently to patients in home and facility-based settings.
