Description

Position: LPN – LTC/Rehab

Facility: ThedaCare Juliette Manor, Berlin, WI

Contract: 8 or 12 weeks; traveler chooses

Start: ASAP

Schedule: 5:30 AM–6:00 PM, 12-hour days

Guaranteed: 36 hours/week

Weekend: Every other weekend

Holidays: As needed

Call: None

Floating: May float to Peabody Manor, about 40 miles away

Ratio: Approximately 1:10

Unit: 20-bed LTC + 17-bed rehab

Orientation: ~5 days (1 onboarding day + 4 preceptor shifts)

Charting: Epic preferred

Medication system: Pyxis highly preferred

Scrubs: Solid colors; professional/lenient policy

Missed shifts: Must be made up within 2 weeks

Local candidates: Accepted

Requirements

Active Wisconsin LPN license
AHA BLS
Associate degree or certificate
2+ years of experience required
Recent LTC/post-acute experience required
Travel experience strongly preferred
Wound care and following MD orders
Blood-draw experience
Epic/Pyxis experience preferred
