Requirements

• Active New York LPN License
• BLS certification – AHA or RQI only
• Minimum 1 year of LPN experience
• High School Diploma or GED
• LTC experience preferred

Job Description

Under the direction of licensed nursing personnel, the LPN provides direct and indirect patient/resident care within the scope of practice. The LPN must demonstrate the knowledge and skills necessary to provide appropriate care to adult and geriatric patients and recognize and report changes in the resident's physical, emotional, and psychosocial condition.

Key Responsibilities

• Provide and document resident care according to the plan of care and facility policies.
• Assist with activities of daily living, including hygiene, grooming, dressing, toileting, incontinence/catheter care, positioning, and skin care.
• Maintain a safe and comfortable resident environment and respond promptly to call lights and alarms.
• Support nutritional needs, including meal delivery, feeding, adaptive equipment, and nourishment.
• Assist with rehabilitation and restorative needs, including range of motion, ambulation, transfers, and proper use of assistive devices.
• Monitor residents and promptly report changes in condition, concerns, refusals, and other relevant information to the Charge/Supervising Nurse.
• Complete all required documentation accurately and in accordance with facility protocols.
• Follow infection-control and safety procedures, including precautions related to potential exposure to blood and bodily fluids.

Experience

• Required: 1 year of experience in LTC or CNA capacity.
• Preferred: 1+ year of experience working as an LPN.
