Description

Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team.

Compensation: Local Pay: $60.00 - $70.00/hour (W-2)

Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes (0.48/mile)

Schedule: Monday–Friday; 8:00 AM–4:30 PM
Productivity: 30 points/week

Key Responsibilities:

Conduct physical therapy evaluations and treatments in patients' homes.
Complete OASIS assessments and documentation.
Develop and implement individualized patient care plans.
Coordinate care with the interdisciplinary team.
Maintain accurate and timely clinical documentation.

Required Qualifications:

Active Physical Therapist (PT) License
HCHB experience required
BLS/CPR certification
Strong communication and time-management skills
