RN Case Manager – Hospice | San Francisco, CA
Weekly Pay: $2500
📍 Location: San Francisco, CA & surrounding areas
🏥 Client: Pathways Home Health and Hospice
📅 Start Date: September 8, 2026
💼 Specialty: Hospice RN
🕐 Shift: Days, 8:30 AM–5:00 PM
📆 Schedule: Flexible based on patient availability
🌙 Weekends: 1 weekend per month
⏳ Duration: 13 weeks

📍 Territory
San Francisco, Brisbane, Daly City, Colma, South San Francisco, Pacifica, and neighboring cities as needed.
🩺 Job Responsibilities
- Admit patients to hospice services
- Assess patients' physical, psychosocial, environmental, safety, and developmental needs
- Develop individualized plans of care
- Document patient observations and interventions
- Coordinate care with a multidisciplinary team
- Conduct follow-up visits as needed
- Participate in hospice quality assessment and performance improvement programs
✅ Requirements
- Minimum 1 year of Hospice RN experience
- Active RN license
- BLS/BCLS required
- Comfortable providing care across the assigned territory
- Strong assessment, care coordination, and documentation skills
