Shift: Night 4x12-Hour (19:00 - 07:30); Day 4x12-Hour (07:00 - 19:30)Description: LTC LVN/LPN*PREVIOUS TRAVEL EXPERIENCE REQUIRED*Must have AK license in handStart Date: ASAPShift: 4x12 Variable (or 3x12 Variable - traveler's choice)-Note that caregivers are not asked to rotate between night/day shifts (occasional exceptions for urgent staffing needs). Shifts will be either Days or Nights, but dependent on facility needs at time of start date.Duration:Traveler's Choice for 26 or 13 week assignmentRatios: 1:20Years of Experience: 1First Timers Accepted: NoCerts REQ: BLSAll caregivers are required to obtain the Alaska Food Worker Certification as a requirement of working in a LTC facility. Clinicans will obtain certification during orientation on-site at the facility. They do not need to complete this prior to their start date.Skills REQ: LTC ExperienceIs on-call REQ: NoWill the clinician float within scope to meet facility needs?: YesOpen to accommodating a block schedule?: NoLocals Accepted: No - Candidates perm address must be at least 50 miles from facilityPending License Accepted: NoRTO Restrictions: No more than 2 weeks of RTO will be considered for a 26 Week Contract, Must be willing to work at least 2 of the 3 major Winter holidays (Thanksgiving, Christmas, New Years). Preference is for traveler's to be willing/able to work any holidays. All RTO must be sent with submittal. The vehicle will either be a facility owned vehicle that will be rented to traveler or a vehicle through a rental car company.Vehicle will be available for use no earlier than 72 hours before contract begins and not longer than 24 hours after contract ends.Only the traveler is authorized to be the operator of the vehicle.Traveler must provide copy of valid driver s license and proof of insurance prior to operating vehicle.Traveler s personal vehicle insurance policy must have rental car insurance to cover any accidents/damages to a rental car.Vehicles for traveler use are provided solely for use around town and should not be taken off-road or outside of 30-mile radius from facility
