Department: Physical Therapist (PT) - Home Health/Hospice Required Certifications/Licensure: BLS required North Dakota Physical Therapy License required Current Driver's License required Current Auto Insurance required Interview Process: Manager interview only No clinical interview involvement Radius Rule: Candidate must live within 50 miles Travelers may perform: Start of Care (SOC) Recertifications Resumption of Care (ROC) Daily patient visits Productivity Requirement: 6.5 points per day Home Vent Experience: Experience Requirements First-time travelers accepted New graduates may be considered if a strong fit Preferred: At least 1 year of experience Home Health experience OASIS training/experience Typical Workload Standard day: 5-6 routine visits per day (when no significant travel or additional assessments/admissions) Admission day: Admission plus approximately 3 additional patient visits Support Available Access to Physical Therapists within sister agencies working similar schedules Clinical Coordinator available for support/questions Director available for support/questions
