Home Hospice RN – Job Summary

Shift: Day, 8-hour shifts | 40 hours/week
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Guaranteed Hours: Yes

Key Requirements

• Minimum 3 years of Home Hospice or Home Health experience
• Active RN license
• BLS certification
• Valid Driver’s License
• Valid Auto Insurance and personal vehicle
• Experience with EPIC Remote Client
• First-time travelers accepted

• Home hospice and end-of-life care
• Pain and symptom management
• Patient and family education
• Care coordination and triage
• Wound care and dressing changes
• Wound VAC management
• Weekly wound measurement and photography
• Drain care: PleurX, nephrostomy, Foley
• End-of-life planning
• Strong organization and time-management skills

Patient Population

• Adults only
• Failure to thrive
• Advanced cancers
• End-stage COPD
• Terminal illnesses and end-stage disease processes

Workload

• Approximately 4–5 patients per day
• Total caseload of approximately 12–15 patients

Territory

Coverage includes areas such as:
Lynwood, Calumet, Calumet City, Calumet Park, Riverdale, and Dolton

Additional Details

• Home visits required
• Quarterly in-person team meetings at Trinity Hospital
• Initial supply pickup at the regional office in Oak Brook
• Phone, laptop, and trunk stock provided
• Other Hospice RNs and management available by phone
• Orientation: NEO plus approximately 1–2 weeks, based on experience and comfort level
• No weekends
• No on-call
• No holidays
• Free parking
• Scrubs: Navy blue or any solid professional color; no graphics, logos, or designs
