Master of Social Work (MSW) required
Minimum 2 years of acute care hospital case management/discharge planning experience
Epic experience required
Travel experience required (Manager may review qualified candidates)
Strong experience with care coordination, discharge planning, post-acute placement, and interdisciplinary collaboration
Experience working with HMOs, IPAs, Medicare, Medicaid, and community resources
Comfortable managing a 20–25 patient caseload in an acute care hospital
Float between units as needed

Complete patient assessments within 24 hours of admission
Coordinate safe and timely discharge planning
Collaborate with physicians, nurses, utilization management, and insurance case managers
Arrange post-acute services, DME, rehabilitation, skilled nursing, and home health as appropriate
Work with a diverse patient population, including adults, geriatrics, infants, and neonates
