Shift:Day 5x8-Hour (08:00 - 17:00)

Home Health Position – Job Details

Work Setting: Home visits with periodic office meetings
Transportation: Reliable transportation required
Driver’s License: Current/valid driver’s license required
Insurance: Agency-required liability insurance required

Patient Volume

Average Patients/Day: 5–8 patients
Types of Cases: Start of Care (SOC), wound care, follow-up visits, and other home health visits

Productivity / Points

Points: 30 points per week
Each visit/activity has a different point value, including:
- Patient visits
- Documentation
- Travel
- Other assigned activities
Contractors are typically assigned 5–10 patients per day, averaging approximately 6–8 points per day

Experience Requirements

Minimum 1 full year of recent Adult Home Health experience, OR
1 full year of Hospice experience, depending on the specialty
First-time travelers: ❌ Not accepted
Must have recent specialty experience

Schedule

Weekend Requirement: Varies by location and profession; discuss during the interview
On-Call: Varies by location and profession; discuss during the interview

Certifications

CPR/BLS: Required

Territory / Travel

Must be willing to travel up to 75 miles from the home clinic
Mileage reimbursement: IRS standard mileage rate
Mileage is reimbursed from patient home to patient home
Travel from home to the first patient and last patient back home is not billable/reimbursable
