Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide treatment to adult and geriatric patients with orthopedic, neurological, and functional mobility diagnoses.
• Perform home health visits and document patient care according to regulatory and reimbursement requirements.
• Maintain productivity expectations of approximately 4.5 visits per day.
• Provide patient education and support safe mobility and functional independence.
• Complete point-of-service documentation using NetSmart EMR.
• Use a laptop for documentation and a cell phone for GPS/navigation and communication.
• Demonstrate strong clinical judgment, independence, organization, and patient-care skills.

Required Qualifications

• Bachelor’s Degree in Physical Therapy – Required
• Physical Therapy licensure for the State of Wisconsin – Required
• NPI number – Required; candidates without an NPI must begin the application process before starting.
• AHA or ARC BLS – Required
• Home Health experience – Required
• Experience working with adult and geriatric populations
• Experience with orthopedic, neurological, and functional mobility diagnoses.
• Travel experience – Required
• Knowledge of Physical Therapy standards and practices.

Travel / Floating Requirements

The primary location is Wausau, with a strong possibility of assisting patients in:

• Antigo
• Rhinelander
• Rapids
• Merrill
• Stanley
• Medford

Mileage: Reimbursed at the government-approved rate.

Weekend Requirements

• PTs rotate one Saturday every 6–8 weeks.
• When working Saturday, the PT receives a weekday off.
• No holiday requirement.
