Description

Position Overview

Seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality nursing care to patients in their homes throughout the Greater New Orleans area. The RN is responsible for assessing patient needs, developing and implementing individualized care plans, coordinating services across the healthcare continuum, and ensuring compliance with home health regulations and agency standards. This role offers the opportunity to make a meaningful impact while working independently in a patient-centered home health environment.

Schedule & Work Details

  • Coverage Area: Greater New Orleans Area
  • Schedule: Monday through Friday
  • Shift: 8:00 AM - 4:30 PM
  • On-Call/Weekend Requirement: None
  • Mileage Reimbursement: Yes
  • Dress Code: Solid-color scrubs preferred (Royal Blue, Black, or Navy)
  • Pay Rate: $51.00 - $53.00 per hour

Key Responsibilities

  • Provide skilled nursing care to patients in their homes as prescribed by the physician and in accordance with agency policies and regulatory requirements.
  • Complete initial, comprehensive, and ongoing nursing assessments to identify patient needs and establish appropriate care plans.
  • Develop, implement, and revise individualized plans of care based on patient condition, physician orders, and clinical findings.
  • Coordinate the total plan of care with physicians, therapists, aides, family members, caregivers, and other healthcare professionals.
  • Promote continuity of care through effective communication and collaboration with all members of the healthcare team.
  • Perform comprehensive patient assessments at designated time points and adjust care plans as needed.
  • Ensure patients meet home health eligibility and medical necessity requirements according to payer and regulatory guidelines.
  • Provide patient and caregiver education regarding disease management, medications, treatments, home safety, and self-care techniques.
  • Utilize point-of-care documentation systems to complete accurate, timely, and compliant clinical documentation.
  • Monitor patient progress, identify changes in condition, and communicate updates to the physician and interdisciplinary team.
  • Participate in care coordination activities and case conferences to support positive patient outcomes.
  • Maintain compliance with Medicare regulations, state requirements, agency policies, and professional nursing standards.

Required Qualifications

  • Active and unrestricted Registered Nurse (RN) license in the state of practice.
  • Current CPR certification.
  • Current driver's license.
  • Reliable transportation and proof of vehicle insurance.
  • Access to a dependable vehicle or public transportation.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, documentation, and organizational abilities.
  • Ability to work independently while managing a field-based patient caseload.

Preferred Qualifications

  • Previous home health nursing experience.
  • Experience with comprehensive patient assessments and care planning.
  • Knowledge of Medicare home health regulations and compliance requirements.
  • Strong patient education and care coordination skills.
  • Experience collaborating with interdisciplinary healthcare teams.
  • Ability to build positive relationships with patients, families, physicians, and community resources.
