Description

Position Overview

Seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients throughout Houghton, Keweenaw, and Baraga counties. The RN will be responsible for managing a home health caseload, conducting comprehensive patient assessments, coordinating care, and delivering skilled nursing interventions in the home setting. This role offers the opportunity to build meaningful relationships with patients while promoting independence, recovery, and overall well-being.

Schedule & Work Details

  • Coverage Areas: Houghton County, partial Keweenaw County, and partial Baraga County
  • Weekly Hour Guarantee: 36 hours
  • Schedule: Monday through Friday
  • Shift: 8:00 AM - 4:30 PM
  • Productivity Expectation: 30 points per week
  • On-Call Requirement: 3 weekdays per month
  • Weekend Requirement: One weekend per month
  • Mileage Reimbursement: Yes, between patient visits
  • Pay Rate: $51.00 - $53.00 per hour

Key Responsibilities

  • Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans.
  • Manage a home health patient caseload and serve as the primary nursing contact for assigned patients.
  • Conduct comprehensive assessments and ongoing evaluations of patient health status.
  • Complete home health admissions, recertifications, resumptions of care, and discharge visits.
  • Perform skilled nursing treatments, including wound care, dressing changes, medication management, and patient monitoring.
  • Educate patients and caregivers regarding disease processes, medications, treatment plans, and home safety.
  • Set up and manage medications as needed to support treatment adherence and positive outcomes.
  • Monitor patient progress and communicate changes in condition to physicians and interdisciplinary team members.
  • Coordinate care with therapists, social workers, home health aides, physicians, and other healthcare professionals.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Participate in case conferences and interdisciplinary care planning meetings.
  • Ensure compliance with agency policies, Medicare regulations, and professional nursing standards.

Required Qualifications

  • Active Registered Nurse (RN) license.
  • Previous home health experience required.
  • Strong clinical assessment and critical thinking skills.
  • Ability to work independently while managing a field-based patient caseload.
  • Excellent communication, organizational, and documentation skills.
  • Valid driver's license and reliable transportation.
  • Ability to travel throughout the assigned coverage area.

Preferred Qualifications

  • Experience with PICC line management and care.
  • Experience with wound VAC therapy and advanced wound care.
  • Knowledge of home health regulations, care coordination, and patient-centered care practices.
  • Strong patient and caregiver education skills.
  • Experience managing complex patients in a home health setting.

Preferred Qualifications

  • Experience with PICC lines and central line management.
  • Experience caring for patients with wound VACs and complex wounds.
  • Strong care coordination and interdisciplinary collaboration skills.
  • Ability to build rapport with patients and families while delivering compassionate, high-quality care.
