Description

LPN – LTC/SNF | Post Acute Nursing

Shift: 2:45 PM–11:00 PM, 8.25-hour shift
Guaranteed Hours: 38 hours/week
Weekend: Every other weekend
Experience: 1+ year LTC experience within the last 3 years
Required: Active LPN license, BLS certification
EMR: PointClickCare (PCC)
Setting: 300-bed LTC/SNF facility serving skilled, rehab, dementia, and hospice residents
Duties: Medication/treatment administration, glucose checks, insulin, IV medications, wound measurement/care, tube feedings, lab draws, catheter/trach care, vitals, oxygen management, ADLs, and documentation.
Scheduling: No call/standby; holiday coverage required as scheduled
