Description
Position Overview
Seeking a compassionate and skilled Registered Nurse (RN) Case Manager to provide high-quality home health services throughout Marquette County and the western portion of Alger County. The RN Case Manager will oversee patient care, coordinate services across the interdisciplinary team, supervise LPN staff, and ensure patients receive the skilled nursing services necessary to achieve optimal health outcomes in the home setting. This position is ideal for a self-motivated nurse who enjoys building relationships with patients while managing care independently in the community.
Schedule & Work Details
- Coverage Area: Marquette County and western portion of Alger County, MI
- Weekly Hour Guarantee: Minimum 32 hours per week
- Schedule: Monday through Friday
- Office Hours: 8:00 AM - 4:30 PM CST
- Productivity Expectation: 30 points per week
- On-Call Requirement: Rotating schedule, 7 days on call followed by 7 days off call
- Mileage Reimbursement: Yes, $0.49 per mile
- Pay Rate: $51.00 - $53.00 per hour
Key Responsibilities
- Manage an assigned home health patient caseload and serve as the primary clinical coordinator for patient care.
- Perform comprehensive nursing assessments and ongoing evaluations in the patient's home.
- Develop, implement, and revise individualized plans of care in collaboration with physicians and other healthcare professionals.
- Coordinate services across the interdisciplinary care team to ensure continuity of care and positive patient outcomes.
- Provide skilled nursing interventions, treatments, and patient education in accordance with physician orders.
- Monitor patient progress and communicate significant changes in condition to the appropriate providers.
- Supervise, mentor, and conduct required oversight visits for Licensed Practical Nurses (LPNs).
- Educate patients and caregivers regarding diagnoses, medications, treatment plans, disease management, and home safety.
- Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
- Participate in case conferences, care planning meetings, and interdisciplinary team discussions.
- Ensure compliance with Medicare regulations, state requirements, and professional nursing standards.
- Respond to patient needs during scheduled on-call rotations.
Required Qualifications
- Active Michigan Registered Nurse (RN) license.
- Strong clinical assessment and patient care skills.
- Valid driver's license and reliable transportation.
- Ability to travel throughout the assigned service area.
- Excellent communication, care coordination, and organizational skills.
- Ability to work independently in a field-based setting.
Preferred Qualifications
- Previous home health experience.
- Experience with Homecare Homebase (HCHB).
- Prior RN case management experience.
- Experience supervising or mentoring LPN staff.
- Knowledge of OASIS documentation and Medicare home health regulations.
- Strong critical thinking and patient education skills.
- Ability to effectively manage a diverse patient caseload while maintaining high-quality care standards.
