Description
Pride Health is actively hiring an Occupational Therapist (OT) to join our client's growing Home Health team.
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Productivity Expectation: 24 points/week
On-Call/Weekend Requirement: None
Location: Farmington Hills, MI, 48334
Coverage Area: (Candidates should be comfortable traveling throughout the assigned coverage area.)

Compensation:
- Local Pay: $53.00–$58.00/hour (W-2)
- Mileage Reimbursement: $0.49/mile, reimbursed between patient visits from the first patient to the last patient.
Position Overview:
We are seeking a dedicated Occupational Therapist (OT) to provide skilled occupational therapy services to patients in their homes. The OT will evaluate patients' functional abilities, develop individualized plans of care, provide therapeutic interventions, educate patients and caregivers, and coordinate care with other members of the home health team.
Key Responsibilities:
- Perform comprehensive occupational therapy evaluations, reassessments, and admission/comprehensive assessments when therapy is included without skilled nursing services; develop and update individualized plans of care based on patient needs and physician orders.
- Provide skilled OT interventions focused on ADL/IADL training, functional independence, fall prevention, home safety, adaptive equipment recommendations, and patient/caregiver education.
- Complete accurate, timely, and compliant clinical documentation while monitoring patient progress and communicating changes in functional status to the interdisciplinary care team.
- Coordinate care with physicians, nurses, PTs, SLPs, social workers, and other healthcare professionals while ensuring services comply with state regulations, Medicare requirements, home health regulations, and Conditions of Participation.
- Provide services in accordance with applicable state regulations, Medicare requirements, home health regulations, and Conditions of Participation.
- Maintain professional standards of care while delivering services in the patient's home environment.
Required Qualifications:
- Graduate of an accredited Occupational Therapy program with a current, unrestricted OT license and CPR/BLS certification; previous home health experience with knowledge of OASIS documentation, Medicare home health regulations, and Conditions of Participation.
- Experience with functional assessments, fall prevention, ADL/IADL training, home safety evaluations, adaptive equipment recommendations, interdisciplinary care coordination, and HCHB (Homecare Homebase) EMR.
