Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) – Home Health

Location: Pittsfield, Massachusetts

Schedule: 8:00 AM – 4:30 PM
Hours: 40 hours/week

Job Details:

• Provide skilled nursing care to patients in their homes based on individualized care plans.
• Perform patient assessments, monitor changes in condition, and communicate significant findings to the appropriate healthcare providers.
• Administer medications and treatments safely and accurately according to physician orders.
• Apply nursing principles and evidence-based practices to promote patient safety and positive outcomes.
• Complete required home health documentation accurately and in a timely manner.
• Perform and maintain proficiency with OASIS documentation and assessments.
• Provide patient and family education regarding medications, disease management, treatments, and appropriate self-care.
• Apply community health principles and practices when developing and delivering patient care.
• Demonstrate the ability to work independently as a clinician with minimal supervision.
• Manage and prioritize an assigned caseload effectively, including self-scheduling when required.
• Adapt to varied assignments and changing patient needs.
• Collaborate with physicians, RNs, therapists, aides, and other members of the interdisciplinary care team.
• Follow all applicable state, federal, facility, and home health regulations and policies.
• Maintain patient confidentiality and adhere to HIPAA requirements.
• Current Massachusetts LVN/LPN license in hand at the time of profile submission.
• Massachusetts license cannot be pending; facility will not pend the license.
• Minimum 1 year of acute nursing care experience; hospital-based experience preferred.
• Previous home health nursing experience – Required.
• Current BLS certification – Required.
• Certified Home Health experience, including proficiency with OASIS, highly preferred.
• Current valid driver's license and reliable vehicle insurance – Required.
• Demonstrated knowledge of nursing principles and practices.
• Ability to work independently with minimal supervision.
• Strong clinical assessment, communication, organization, and time-management skills.
