Description
Position Overview
Seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients throughout Collier County, FL.
Schedule & Work Details
• Coverage Area: Collier County, FL
• Schedule: Full-time, 40 hours per week
• Shift: Monday through Friday, 8:00 AM - 5:00 PM
• On-Call/Weekend Requirement: As needed
• Mileage Reimbursement: Yes
• Patient Care Setting: Patient homes throughout the service area
• Pay Rate: $51.00 - $53.00 per hour
Key Responsibilities
• Provide skilled nursing visits to patients in their homes in accordance with physician orders and individualized care plans.
• Complete OASIS assessments, including:
o Start of Care (SOC)
o Resumption of Care (ROC)
o Recertification Visits
o Discharge Assessments
• Deliver skilled nursing interventions, including wound care, ostomy care, Foley catheter care, and other clinical treatments as ordered.
• Manage a daily patient schedule while traveling throughout the assigned coverage area.
Required Qualifications
• Active Registered Nurse (RN) license in the State of Florida.
• Strong clinical assessment, organizational, and time-management skills.
• Excellent verbal and written communication skills.
Preferred Qualifications
• Previous home health experience.
• Experience completing OASIS assessments and documentation.
• Ability to work independently and effectively manage a field-based caseload.
