Registered Nurse – Home Health | Denver, CO
Pride Health is hiring a Registered Nurse – Home Health for a 14-week contract assignment supporting a leading healthcare organization in Denver, CO.
Responsibilities Include:
• Provide skilled nursing care to home health patients, including wound care, catheter management, medication management, and patient education.
• Perform Start of Care (SOC) visits and complete OASIS assessments and documentation.
• Manage patient care plans, coordinate services, and monitor patient progress as an RN case manager.
• Collect and manage labs while providing ongoing support and education to patients and families.
Qualifications:
• Active Registered Nurse (RN) license required.
• Home health nursing experience required.
• Experience completing OASIS assessments and Start of Care visits required.
Important Details:
• Assignment Length: 14-week contract
• Start Date: ASAP
• Schedule: Monday–Friday, 8:00 AM–4:30 PM
• Hours: 8-hour shifts / 40 hours per week
• On-Call Requirement: Rotating on-call schedule
• Coverage Area: Denver Metro Area
• Mileage Reimbursement: Yes, mileage reimbursed between patient visits
• Pay Range: $52.00–$54.00/hour
