Integrated Care Nurse Practitioner – Primary Care

Location: Fresno, CA
Additional Location: Clovis, CA
Compensation: $120,000–$145,000 annually
Schedule: Full-Time | 40 hours/week

We are seeking an Integrated Care Nurse Practitioner to provide personalized, value-based care to patients living with chronic kidney disease (CKD) and end-stage renal disease (ESRD).

This is a field-based, community-focused position involving in-home and virtual patient visits. The NP will conduct comprehensive assessments, annual wellness visits, chronic disease management, preventive care, medication management, and care coordination.

The provider must reside within 30 miles of Fresno, CA and be comfortable traveling throughout the assigned territory.

Key Responsibilities:

• Conduct comprehensive annual wellness visits in patients' homes and virtually
• Perform health assessments and develop individualized care plans
• Prescribe medications and order diagnostic testing
• Manage chronic conditions and provide preventive care
• Educate patients and families regarding disease management
• Coordinate care with PCPs, specialists, nurses, and community health workers
• Identify and close gaps in care
• Maintain accurate and timely EMR documentation
• Participate in integrated care team meetings
• Support quality improvement, patient outcomes, and value-based care initiatives

Requirements:

• Active, unrestricted California RN and NP license
• ANCC or AANP board certification
• Current, unrestricted DEA
• Minimum 2 years of direct patient-care experience
• Valid driver's license, reliable transportation, and auto insurance
• Ability to practice autonomously
• EMR and point-of-care technology proficiency
• Knowledge of HIPAA, OSHA, and applicable regulations

Preferred:

• Managed Care, IPA, or Health Plan experience
• Annual Wellness Visit experience
• Home-based or community-based care experience
• CKD/ESRD experience
• Value-based care experience
• Bilingual Spanish
