Case Manager RN – Inpatient Case Management
Specialty: Case Management
Shift: Day, 4x10-hour shifts, 8:00 AM–6:30 PM
Schedule: Thursday–Sunday
Contract: 13 weeks

Job Overview
We are seeking an experienced Registered Nurse Case Manager to provide inpatient case management services at a community-based hospital. The Case Manager will assess patient needs, coordinate multidisciplinary care, develop individualized treatment and discharge plans, and support safe, efficient transitions across the continuum of care.
The ideal candidate will have strong inpatient case management experience, excellent care-coordination skills, and the ability to work independently in a hospital environment.
Required Qualifications
- 2 years of RN experience required.
- Minimum 1 year of inpatient Case Management experience required.
- Outpatient-only case management experience does not meet the requirement.
- Current RN license required.
- BLS certification required.
- First-time travelers are accepted.
- Previous charge experience preferred.
- Acute hospital experience preferred.
- Cerner experience required.
- Non-local candidates only; no candidates within 100 miles of the facility.
- Non-local driver's license must be on file.
- New contracts and extensions must be at least 10 weeks.
Key Responsibilities
- Perform comprehensive inpatient assessments and identify patient care needs.
- Develop and coordinate individualized plans of care.
- Evaluate treatment options and patient progress.
- Coordinate services across the interdisciplinary care team.
- Facilitate discharge planning and safe transitions of care.
- Coordinate post-acute services, referrals, and follow-up care.
- Advocate for patients and families throughout the care process.
- Collaborate with physicians, nursing staff, social services, physical therapy, pharmacy, rapid response teams, and other healthcare professionals.
- Monitor utilization and promote appropriate, cost-effective care.
- Document case management activities accurately in Cerner.
- Support quality outcomes, patient satisfaction, and continuity of care.
- Assist patients in navigating healthcare resources and services.
Patient Population & Environment
The hospital is a community-based hospital and includes a Level III Trauma Center and Level I Teaching Facility.

Daily Census: Approximately 60
Case Management Ratios: 1:5 days; 1:6 nights/weekends
Charting: Cerner
Orientation: 16 hours
