Position: Social Worker
Shift: Days – 8:00 AM–4:30 PM
Schedule: 5 × 8-hour shifts
Guaranteed Hours: 40 hrs/week
Weekend: 2 weekends/month
Holiday: Every other winter and summer holiday required
EMR: Cerner & AllScripts
Patient Ratio: 1:18 days / 1:50 weekends

Responsibilities

• Provide Medical Social Work services in the Emergency Department.
• Conduct initial evaluations and biopsychosocial assessments.
• Develop plans of care based on patient and caregiver needs.
• Coordinate discharge planning, home care, equipment, and community resources.
• Collaborate with multidisciplinary teams and participate in care conferences.
• Coordinate referrals to rehabilitation, long-term care, and other appropriate services.
• Provide support with advance directives.
• Document assessments, interventions, and care plans.

Requirements

• 3 years of Social Work experience – Required
• Acute Care / Inpatient Medical Social Worker experience – Required
• MSW – Required
• Maryland LMSW or LCSW – Required
• Valid Maryland or Compact license – Required
• Advance Directive experience – Required
• First-time travelers accepted.
