Facility Size: 70 Beds
Care Setting: Skilled Nursing & Assisted Living
Schedule: Full-Time

About the Opportunity

A 70-bed skilled nursing and assisted living facility is seeking a dependable and compassionate LPN for the Day shift.

This is a great opportunity for an LPN who enjoys working in a smaller, community-focused facility and is looking for a stable overnight nursing position with the opportunity to make a meaningful impact on residents.

Responsibilities

• Provide nursing care to skilled nursing and assisted living residents during the overnight shift
• Administer medications and treatments according to physician orders
• Monitor resident conditions and recognize changes in health status
• Complete accurate and timely clinical documentation
• Communicate changes in condition to the appropriate provider, DON, or on-call leadership
• Assist with resident assessments and care planning
• Provide compassionate, individualized resident care
• Work collaboratively with CNAs and other members of the care team
• Follow facility policies and state and federal regulations
• Respond appropriately to overnight resident needs and emergencies

Qualifications

• Previous long-term care or skilled nursing experience preferred
• Strong clinical and communication skills
• Ability to work independently and prioritize resident needs during the overnight shift
• Dependable, compassionate, and team-oriented approach to patient care
