• 60-minute initial evaluations
• 30-minute follow-up appointments

Position Overview

Seeking an experienced Physical Therapist for an outpatient contract assignment. The PT will evaluate assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction.

Key Responsibilities

• Complete comprehensive patient evaluations
• Develop functional treatment plans and measurable goals focused on patient progress and discharge
• Provide and appropriately delegate discipline-specific therapy services
• Design treatment programs that promote a safe and effective return to community activities
• Educate patients and families regarding treatment plans and progression
• Complete accurate and timely clinical documentation
• Document goals, functional assessments, medical necessity, and charges as required
• Ensure treatment is provided under appropriate physician orders and insurance authorization
• Follow applicable state practice standards and regulatory requirements
• Participate in quality improvement and patient-safety initiatives
• Maintain a safe treatment environment and follow infection prevention and safe patient-handling practices

Requirements

• Current Physical Therapist licensure in good standing
• Ability to evaluate patients independently and develop appropriate plans of care
• Strong clinical documentation and patient education skills
• Ability to work collaboratively with the interdisciplinary team
• Must meet applicable continuing education and professional practice requirements
