Job Details:
Schedule: Monday–Friday | 8:00 AM–4:30 PM | 40 Hours/Week
Location: Brewton, AL
Job Type: Local Contract
Contract Length: 13 Week/s
Duties:
- Perform physical therapy admissions, evaluations, and re-evaluations for home health patients.
- Develop and implement individualized treatment plans based on patient needs and functional goals.
- Provide skilled physical therapy interventions to improve mobility, strength, balance, gait, and overall function.
- Perform physical therapy discharge visits and assess patient progress toward established goals.
- Educate patients and caregivers on therapeutic exercises, mobility, safety, and home-care strategies.
- Maintain accurate and timely clinical documentation and coordinate care with the interdisciplinary team.
Requirements:
- Education: Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
- License(s):Active Alabama Physical Therapist License – Required
- Certification(s): BLS
- Years of experience: Previous Home Health Physical Therapy experience – Required
- Skills/Specialties/Technology: Home Health PT, PT Admissions, Evaluations, Re-evaluations, Discharges, patient/caregiver education, clinical documentation, and ability to work independently in a home-based setting.
