Home Health RN – Pittsfield, MA | Berkshire Health System

Location: Pittsfield, Massachusetts
Specialty: Home Health / Community Health RN
Schedule: Flexible based on caseload
Weekends: Every 3rd weekend
Holidays: 1–2 holidays; coverage as needed
On Call: None
Orientation: 8 hours non-billable
Local Rule: Candidates residing within 50 miles of the facility will not be considered

Position Summary

The Community Health Nurse provides skilled nursing care to patients in their homes, with a focus on maintaining or restoring health, preventing illness, and supporting rehabilitation. This role requires strong independent clinical judgment, critical thinking, time management, and the ability to manage a home health caseload with minimal supervision.

Required Experience

• 1+ year of acute care nursing experience
• Hospital-based acute care experience preferred
• Home Health experience required
• Certified Home Health experience required
• OASIS proficiency required
• Strong understanding of home health reimbursement and regulatory guidelines
• Ability to independently manage patients and caseload
• Self-scheduling/caseload management experience preferred

Certifications & Licensure

• Active Massachusetts RN license required at submission
• AHA BLS required
• Infusion Therapy Certification preferred
• Valid driver's license and auto insurance required

Traveler Requirements

• Traveler must have their own vehicle
• Must be comfortable driving in inclement weather
• Permanent address must be included with submission
• Facility prefers travelers with a 45-minute-or-less commute from 165 Tor Ct, Pittsfield, MA 01201
• Candidate cannot reside within 50 miles of the facility

Submission Requirements

Profile must include:

• Current Home Health RN skills checklist completed within the past year
• 1 supervisory signed reference from employment within the past 2 years
• Massachusetts RN license
• Active AHA BLS
• Complete work history
• Permanent home address
• All requested time off
• At least 2 specific interview dates/times

“Anytime” will not be accepted for interview availability.

RTO / Scheduling

• Maximum 5 days RTO; fewer preferred
• All RTO must be disclosed at submission
• All scheduling requests must also be submitted upfront
• RTO or scheduling requests added during the interview or after submission will not be approved
• Late scheduling requests may result in the offer being rescinded
