Description
Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.
Pay rate: Up to $55.00/ hour (W-2)
Work Location: Austintown, OH, 44515
Weekly Hour Guarantee: 36 hours/week
Patient Care Setting: Home Health
Schedule: Monday–Friday: 8:00 AM – 4:30 PM
On-Call: 2–3 times per week
Weekend: Every other weekend (Saturday & Sunday) with on-call responsibility
Mileage reimbursement provided between patient visits, from the first patient to the last patient ($0.49/Mile)
Coverage Area: Mahoning County, Columbiana County, Trumbull County, Some areas of Portage County (Garrettsville, Windham, Deerfield, Atwater, Nelson, Palmyra, Paris)
Key Responsibilities:
- Provide high-quality direct patient care in the home health setting.
- Perform comprehensive nursing assessments and monitor patients' conditions.
- Develop and implement individualized patient care plans.
- Complete accurate and timely clinical documentation and charting.
- Meet established productivity expectations and manage an assigned patient caseload effectively.
- Participate in on-call and weekend rotations as required.
- Attend weekly case conferences and clinical/team meetings.
- Maintain frequent daily communication with clinicians, physicians, patients, caregivers, and the office team.
- Identify changes in patient condition and communicate appropriately with the care team.
- Coordinate care and support continuity of services across the patient's home health episode.
Required Qualifications:
- Current Registered Nurse (RN) license.
- Home health nursing experience preferred.
- Strong clinical assessment, patientcare, and documentation skills.
- Ability to work independently while collaborating effectively with the clinical and office teams.
- All required clinical skills and certifications must be current.
Preferred Qualifications:
- Previous experience with Homecare Homebase (HCHB) charting system.
- Strong home health and care coordination experience.
- Excellent communication, organization, and time-management skills.
