Travel Physical Therapist (PT) – Home Health / Skilled Nursing
Sarasota, Florida
We are seeking a Travel Physical Therapist (PT) for a 13-week contract in Sarasota, Florida, working with patients in a skilled nursing setting with a home-health-focused approach. This is a great opportunity for a PT who enjoys working with a geriatric population, promoting functional independence, and helping patients safely return to their prior level of function.

- Experience: SNF or home health experience preferred
Responsibilities
- Evaluate patients and develop individualized plans of care
- Provide skilled physical therapy interventions focused on mobility and functional independence
- Treat patients with a variety of medical, orthopedic, neurological, and age-related conditions
- Provide gait, balance, transfer, strengthening, and mobility training
- Assist patients with safe use of assistive devices
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with nursing, therapy, and other members of the interdisciplinary team
- Complete timely and accurate clinical documentation
- Educate patients and caregivers on safety, mobility, and home exercise programs
