Schedule: Full Time, 40 Hours/Week

Shift: Monday-Friday, 8:00 AM-4:30 PM

Travel Package: $3,076/Week
 

Position Overview

We are seeking a Physical Therapist (PT) for a 13-week assignment primarily supporting a Skilled Nursing Facility (SNF)/Extended Care Center (ECC). The PT will provide rehabilitation services to both short-term rehab patients and long-term care residents. Based on departmental needs, the therapist may also assist within the hospital's inpatient acute care setting.

Responsibilities

• Evaluate and treat SNF rehabilitation patients and residents.
• Develop and implement individualized therapy plans.
• Collaborate with Occupational Therapy and the interdisciplinary care team.
• Participate in weekly patient care conferences.
• Contribute to discharge planning and care recommendations.
• Maintain accurate documentation in EPIC and Therapute.
• Flex to the inpatient department as needed.

Caseload

• Up to 10 patients per day
• No templated schedule or fixed treatment times
• Mix of rehab and long-term care residents

Requirements

• Active Physical Therapist license
• Minimum 1-2 years of PT experience
• Skilled Nursing Facility and rehabilitation experience preferred
• Strong interdisciplinary communication skills
