Description
Shift Days (M-F, weekends): M-F with one weekend a week
Shift Hours (start/end time): 8-5; in the ED some 7p-3:30a shifts
6 core responsibilities:
• Conducts psychosocial assessments and provides appropriate interventions for patients and families.
• Provides crisis intervention, advocacy, education, and community resource navigation.
• Collaborates with interdisciplinary teams to address barriers and support safe transitions of care.
• Participates in discharge planning, care coordination, rounds, and complex case consultation.
• Maintains timely and accurate clinical documentation.
• Provides comprehensive inpatient social care coverage across services and campuses based on operational needs.
Candidate:
MUST have prior Level 1 or 2 Trauma Center experience.
