Location: Woodhaven, New York
Join Our Team
Hands of Hope Physical Therapy is seeking a dedicated, compassionate, and motivated Physical Therapist (PT) to join our outpatient rehabilitation team in Woodhaven, NY. This is an excellent opportunity to work alongside an experienced multidisciplinary team, including our pain management department, to deliver exceptional, patient-centered care.
As a Physical Therapist, you will assess, develop, and implement individualized treatment plans that help patients restore mobility, reduce pain, improve strength, and maximize functional independence. You'll have the opportunity to make a meaningful impact on patients recovering from orthopedic injuries, chronic pain, post-surgical conditions, and other musculoskeletal disorders while working in a collaborative clinical environment.
Key Responsibilities
- Perform comprehensive physical therapy evaluations to assess patients' physical condition, functional abilities, mobility limitations, and rehabilitation needs.
- Develop individualized treatment plans based on clinical findings, physician referrals, and patient-centered goals.
- Deliver evidence-based therapeutic interventions, including therapeutic exercise, manual therapy, neuromuscular re-education, electrical stimulation, ultrasound, and heat/cold modalities.
- Educate patients and caregivers on home exercise programs, injury prevention, body mechanics, and self-management strategies.
- Monitor patient progress through ongoing assessments and modify treatment plans to optimize functional outcomes.
- Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR) system.
- Collaborate closely with physicians, pain management providers, and interdisciplinary healthcare professionals to coordinate comprehensive patient care.
- Participate in continuing education and professional development to remain current with evidence-based physical therapy practices.
- Adhere to HIPAA regulations, company policies, ethical standards, and all applicable professional practice guidelines.
Qualifications
Required
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
- Active Physical Therapist (PT) license in the State of New York.
- Strong clinical reasoning, communication, and interpersonal skills.
- Ability to work independently while contributing effectively to a multidisciplinary healthcare team.
- Compassionate, patient-centered approach to rehabilitation.
Preferred
- CPR/BLS Certification.
- Previous experience in an outpatient rehabilitation clinic.
- Experience treating orthopedic, neurological, post-surgical, and chronic pain patients.
- Familiarity with WebPT or other Electronic Medical Record (EMR) systems.
Physical Requirements
- Ability to stand, walk, bend, kneel, and perform physical therapy techniques for extended periods.
- Ability to safely lift, transfer, and assist patients with mobility as needed.
- Ability to operate therapeutic equipment and treatment modalities safely and effectively.
