Description
Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.
Weekly Gross Pay: Up to $2,472.00/ hour (W-2)
Schedule: Monday–Friday, 8:00 AM–4:30 PM EST
Weekly Hours: 32 hours guaranteed
Productivity: 24 points/week
On-Call: 1 day/week + 1 weekend/month
Mileage: $0.51/mile between patient visits
Scrubs: Navy Blue
Certification: BLS required
EMR: HomeCare HomeBase (HCHB/PointCare)
Work Location: Farmington Hills, MI 48334
Coverage Area: Oakland, Wayne, Livingston, Washtenaw, Macomb, and Genesee Counties, within the designated service zone.
Key Responsibilities:
- Perform comprehensive in-home assessments, including vitals, medications, ADLs, home safety, and overall patient condition.
- Develop and update individualized plans of care based on patient needs and physician orders.
- Provide skilled nursing services including medication administration, wound care, IV therapy, and post-hospital/surgical care.
- Monitor patient progress, identify changes in condition, and communicate concerns to physicians and the care team.
- Educate patients and caregivers on medications, disease management, safety, infection prevention, and self-care.
- Coordinate care with physicians, therapists, social workers, aides, and other healthcare professionals.
- Complete accurate and timely clinical documentation while maintaining Medicare/CMS and agency compliance.
- Provide patient advocacy, emotional support, and independent clinical decision-making in the home setting.
Required Qualifications:
- 1+ year of Home Health RN experience.
- 1+ year experience using HomeCare HomeBase (HCHB/PointCare) EMR.
- Active, unencumbered RN license in the state of practice.
- Current BLS certification.
- Ability to work independently and make sound clinical decisions in patients' homes.
- Valid driver's license and reliable transportation.
- Preferred Skills: Strong clinical assessment, medication/treatment management, wound and post-acute care, communication, time management, and organization.
