License Requirements:
- Active Maryland PT License or Compact State PT License required
Requirements
- Home Health experience preferred
- Oasis documentation experience required
- Homecare Homebase (HCHB) EMR experience required
- Strong customer service and time management skills
Key Responsibilities
- Evaluate and treat patients based on physician orders and referrals.
- Develop individualized treatment plans with measurable goals.
- Monitor patient progress and modify treatment plans as needed.
- Communicate patient needs and progress with physicians, patients, families, and the care team.
- Recommend adaptive equipment to improve patient function and independence.
- Complete required clinical and billing documentation.
- Supervise Physical Therapist Assistants and Physical Therapy Aides as required.
