Case Manager LVN – Days
📍 Location: Woodland Hills, CA
📅 Start Date: 9/21
🕐 Shift: Days | Monday–Friday, 8:00 AM–4:30 PM
📋 Schedule: 5×8s | 40 hours/week
⏳ Contract: 13 weeks
💰 Weekly Gross: $2,000/week
Position Details
- Support patients and families throughout the hospital discharge process
- Assess patient and caregiver readiness for discharge
- Round on patients and communicate needs or concerns to the RN
- Reinforce discharge education and ensure patients/families have necessary resources
- Identify barriers to discharge and collaborate with the care team to resolve them
- Help support safe and efficient patient flow and transitions of care
- Serve as a liaison between patients, families, and the RN
- Each LVN will support a Med Tele unit
- Units have approximately 24 beds, with caseloads generally ranging from 20–24 patients
- Floating is required based on staffing needs. Candidates must be comfortable floating as needed.
- No on-call requirement
- Local travelers accepted
Requirements
- Active CA LVN License
- 2+ years of LVN experience
- Experience in Case Management and/or Med Tele
- BLS
- COVID vaccine not required
RTO
- Up to 5 days of RTO may be accepted
- Candidates with more than 5 total RTO days will not be accepted
💰 Travel Pay Package
- $25/hr taxable
- $1,000/week housing stipend
- $2,000 gross/week
