Description
Registered Nurse – Case Manager
- Provide case management services for acute care patients.
- Perform utilization management and coordinate appropriate levels of care.
- Collaborate with physicians, nurses, social workers, and multidisciplinary teams.
- Develop and monitor individualized patient care plans.
- Coordinate admissions, discharges, transfers, and post-acute care needs.
- Review medical records and ensure appropriate utilization of healthcare services.
- Support care transitions and facilitate appropriate referrals and resources.
- Maintain accurate clinical documentation in Epic EMR.
- Experience: 3+ years of acute care Case Manager experience with utilization management experience required.
- License: Active California RN License required.
- Certifications: BLS required.
- Shift: Day, 5x8s (8:00 AM – 5:00 PM).
- Weekend: No | Floating: No | On-Call: No
