Description
Department: Social Work
Shift: 8-hour Days
Schedule: 40 hours/week
Weekend Requirement: Every Saturday
Contract Length: 13 weeks
Guaranteed Hours: 40 hours/week
EMR: Cerner
License Required: YesMust-Have Requirements
- Recent CA Dignity Health experience – Mandatory
- MSW degree from a CSWE-accredited school
- 2+ years clinical/medical social work experience
- BLS certification
- 5150 certification / ability to obtain county certification
- Active license required at submission
- 2 supervisory references required at submission
- ED/Acute Care experience preferred
Position Responsibilities - Perform social work assessments and interventions for inpatient, outpatient, and Emergency Department patients
- Provide crisis intervention, patient/family support, and brief counseling
- Conduct high-risk screenings
- Assist with financial, community, and other resource referrals
- Coordinate complex discharges and post-acute placement
- Develop and implement patient-centered treatment plans
- Collaborate with physicians, case managers, nursing staff, and other healthcare professionals
- Facilitate patient and family meetings with the healthcare team
- Engage appropriate county agencies and community resources
- Maintain professional relationships with patients, families, healthcare teams, and community agencies
- Participate in hospital committees and staff education/in-service programs
