Requirements
- Active LCSW in state of CA
- Minimum 5 years of LCSW experience
- Epic EMR experience required
- Recent experience in outpatient behavioral health
- Strong psychosocial assessment, counseling, care coordination, and resource management skills preferred
- Experience working within ambulatory, primary care, family medicine, or integrated behavioral health settings is a plus
Position Overview
Seeking an experienced LCSW to support an outpatient Primary Care/Family Medicine practice in Palo Alto, CA. The social worker will provide behavioral health support, psychosocial assessments, care coordination, patient advocacy, crisis intervention, counseling, and community resource referrals. The role requires collaboration with physicians, nurses, and interdisciplinary care teams to support patients' behavioral health and social service needs.
Compensation
Weekly Pay Package: $2,688/week
- Taxable Hourly Pay: $1,000/week
- Stipends: $1,688/week
